DS conducts Health Inequalities Survey

 Community Dental Services CIC, along with Healthwatch Central Bedfordshire (HWCB) and The Disability Resource Centre (DRC), has been commissioned by Bedfordshire, Luton and Milton Keynes Integrated Care System (BLMK ICS) to conduct an independent patient experience survey. 

The survey aims to determine the nature, causes and potential solutions to health inequalities for people with physical and/or learning disabilities, who live in areas of high need across BLMK. The feedback received from this survey will help to improve services and better meet expectations of people across BLMK. 

Patients with a physical and/or learning disability and live in Bedfordshire, Luton or Milton Keynes, are invited to complete this short multiple choice survey and share their experiences of the health and/or social care services received, or are currently receiving.  


A Carer/relative/friend etc can complete this survey on behalf of the patient if preferred, and all answers will remain completely anonymous.
